Vision Benefits for Healthflex Members

Have you taken advantage of your vision benefits yet? HealthFlex members have two types of vision coverage: Vision Service Provider (VSP) and EyeMed Vision Care.

VSP

No ID cards or claim forms are necessary. Find a VSP network doctor at www.vsp.com or call 1-800-877-7195. Make an appointment and tell the doctor you are a VSP member. Your doctor and VSP will handle the rest.

Benefits:

• $10 co-pay on one eye exam per year
• 20% discount on prescription glasses (lenses and frames)
• 15% discount on contact lens fitting and evaluation exam
• Exclusive pricing on annual supplies of popular contact brands
• Laser vision correction discounts
• When visiting an out-of-network provider, receive up to a $25 reimbursement on eye exam

EyeMed

Just present your BCBS ID card. Log onto www.bcbsil.com or call 866-273-0813 to locate a provider.

Benefits:

• $5 off unlimited routine exams
• $10 off unlimited contact lens exams
• 45% off regular priced frames up to $130, plus an additional 20% off the balance over $130
• 15% discount on contact lenses
• Additional discounts on various lens options
• Laser vision correction discounts

You will notice, VSP pays better on exams, and EyeMed pays better on lenses and frames. It is possible to use both. Your prescription belongs to you. You can have your eyes examined at a VSP provider and take your prescription to an EyeMed provider to purchase your lenses and frames.
